Appointments

​​There is a $35 fee if your appointment is cancelled within 24 hours. No shows will be charged a $35 fee, no exceptions.

Insurance

​Call the office in advance at (646) 421-6064 to confirm that your subtype of insurance is accepted and check with your insurance plan to determine whether or not you need a referral or preauthorization for Mohs surgery (codes 17311, 17312, 17313, 17314, 17315).

Generally the following plans are accepted:
Medicare, Cigna, Oxford, United Healthcare, GHI, Aetna, HIP, 1199, Multiplan, Empire Bluecross Blueshield
